Microsoft Excel software under a Windows environment is required to use this template
These Excel survey with option buttons work on all versions of Excel since 2007.

To be able to use these models correctly, you must first activate the macros at startup.
The file to download presents three models Excel survey with different option buttons:

Instructions :
- To run the SetupSurvey macro, on the Ribbon, click the View tab, then click Macros
Select the SetupSurvey macro, and click Run.
- After creating the survey:
-- Weighting in column B can be changed from 1 to any value.
-- You can format the worksheet, and change the text
-- To clear a set of option buttons, delete the score in column C for that row.
-- Survey total is shown in cell A1.
- To run the SetupSurvey2 macro, choose Tools | Macro | Macros OR, on the Ribbon, click the View tab, then click Macros
Select the SetupSurvey2 macro, and click Run.
You will be prompted for the number of questions in the survey.
This creates a survey form with Responses valued 0-4, and an N/A response.

- After creating the survey:

-- You can move the Survey sheet to another workbook, as the macro is only required for setup.

Select the SetupSurvey3 macro, and click Run.
You will be prompted for the number of questions in the survey.
This creates a survey form with Responses valued 0-4, and an N/A response.

-- Range M2:M7 is named RespList
-- Range N2:N7 is named ScoreList
